Please fill in the information required on the reverse and detach the Contributor form. Enclose your contributor form and donation in the addressed envelope provided and mail or drop off at either facility.

Please make cheques payable to HHHS Foundation

An official income tax receipt will be sent to the contributor.

The Special Occasion Card may be given to your honouree or mailed in the envelope provided.
