Acute & Ambulatory Services  

 Both Minden and Haliburton sites provide 24 X 7 Emergency Care.  The Haliburton site provides

the County with a 14-bed Acute Care unit, and one room especially designed for planned births. 

 "State-of-the-Art" facilities at both Minden and Haliburton (constructed in 2000) continue to

receive improved and new equipment through donations by the Hospital Auxiliaries and the HHHS

Foundation.

Long Term Care

Hyland Crest and Highland Wood are Long Term Care facilities providing RN, RPN, and PSW

services to the residents.  Operated and funded according to Ministry of Health & Long Term Care

standards, the two nursing homes provide optimal care to the community's elderly folk.  Daily

activities, nutritional meals, planned outings and visiting entertainers help to ensure well-rounded

events for those who choose to participate.  Privacy is assured within the one and two room suites,

which offer telephone and cable T.V. services.

Haliburton Highlands Mental Health Services 705-286-4575  

 

The Haliburton Highlands Mental Health Services (HHMHS) opened its doors in 1996. 

Funded through the Ministry of Health & Long-Term Care, it offers a range of mental health

services, including: 

  • crisis response to consumers engaged in the program 
  • general individual, couple and family counselling 
  • psychotherapy and intensive case management
  • family support
  • multi-agency coordination of services
  • Homeless Initiative 
  • Early Psychosis Intervention Program (EPI/LYNX)

The program serves approximately 350 active clients yearly, and provides up to 1200 individual

counselling sessions per annum. Under the auspices of HHMHS, a Psychiatric consultation is

offered regularly by a visiting specialist.

 

 

Homeless Initiative

The Halliburton Highlands Mental Health Services facilitates  housing and case management

support to residents of the Haliburton Highlands, suffering from serious mental illness and

considered to be at risk of being homeless.

 

Early Psychosis Intervention Program (EPI/LYNX)

EPI/LYNX offers assessment, treatment, support, and education for people aged 14 - 45

experiencing the early stages of psychosis, and their families.

Haliburton Highlands Supportive Housing Services 457-1392 Ext. 249 or 286-2140 Ext. 295

Since 1998, the Haliburton Highlands Supportive Housing Services has been providing care to

clients in their homes.  These individuals live in a clustered format, within 3 - 4 minutes of the

staff offices at HHHS.  Services include personal care, home management, rehabilitation and

nutritional management.  The goal is to augment an individual's independence, delaying the need to

be admitted into a Long-Term Care facility.  Services are available from 7:30 a.m. to 10:00 p.m.

with 24-hour emergency response.  The program is securely partnered with the Community Care

Access Centre and networked to various other community health care services.

 

Haliburton Highlands Diabetes Education Network 286-2140 X 296 or 457-1392 X241

The diabetes education team is made up of a Registered Dietitian, two Registered Nurses and

Clerical Support. We will work together with you, your doctor and other health care members to

help manage your diabetes.   

 

Family Health Team (FHT)   www.hhfht.com  

HHHS is pleased to be a key leader in the development and governance of the Haliburton Highlands

Family Health Team (FHT).  A FHT is a model of primary care delivery that joins physicians with

allied health professionals such as Nurse Practitioners, Registered Nurses, Social workers,

Dietitians and chiropodists to provide a full basket of primary care services that will meet the

needs of the entire community.  Connecting with this team will be a variety of specialists who will

 improve access to specialist care and consultations.  The Haliburton Highlands FHT was approved

 in April of 2005 and continues to develop.  It is now incorporated and making preparations to

Life Labs (formerly MDS Laboratory Services)

MDS Services are located in the Clinic area of the Community Health Centre space at the Minden

site, as well as in the Haliburton Highlands Health Centre, next door to HHHS Haliburton 

This partnership provides bloodwork services to the area, although some tests are still conducted

at larger centres.
