Family Health Team

The Ministry of Health & Long Term Care has funded a Family Health Team (FHT) in the Haliburton Highlands.  A partnership between the HKPR Community Care Access Centre (CCAC), the Haliburton Highlands Health Services (HHHS), and the Haliburton Family Health Network (Haliburton physicians) has developed gradually, and is now providing health care to residents of Haliburton County.  A group of health professionals, including physicians, nurse practitioners, registered nurses and health specialists work closely with each other and with community partners to provide service to people of the Highlands. The Haliburton FHT is located in the new medical centre called Haliburton Highlands Health Centre, and the Minden FHT offices are located in the second story, above the Minden Emergency Hospital.

Goals of the Family Health Team

·        To meet the primary health care needs of the residents in the Haliburton Highlands.

·        To improve access to primary health care and close gaps in the services.

·        To reduce barriers to healthy living and primary health care for the residents of Haliburton Highlands.

·        To enhance the co-ordination and integration of health care within Haliburton Highlands.

·        To coordinate and facilitate health referrals outside of Haliburton Highlands.
