HHHS provides
Ambulatory/Emergency Care, Long-Term Care and Community Health programs
including the Diabetes Education
Network, a Mental Health program and a Supportive Housing Service.
The Haliburton site
of HHHS includes 13 patient beds & one maternity bed in their Acute
Care and Emergency Departments, serving both in-patient and out-patient requirements on a 24 X
7 basis. X-ray,
Physiotherapy, and the main administration offices are located at
this site, as well as satellite offices for the Programs. Highland
Wood, a 30-bed LTC facility is
located at the Haliburton site of HHHS. Contact 705-457-1392
Ext.221 for further information.
HHHS
Minden site is open 24 hours
a day, seven days a week. The Emergency Department stabilizes trauma
patients prior to dispatching to larger centres, as well as providing X-ray
and Physiotherapy services by appointment. Program
offices for HHHS Supportive Housing and
Diabetes Education are located on the second
floor in the Community Health area. Hyland Crest 62-bed LTC facility is an
integral part of the Minden
facility. Contact: 705-286-2140 Ext. 221 for these

The Haliburton Highlands
Family Health Team (FHT) and the Minden
Medical Clinic are both located above the Emergency Department in the Minden site of HHHS,
operating independently to provide primary care delivery. The FHT
joins physicians with allied health professionals such as Nurse
Practitioners, Registered Nurses, Social workers, Dietitians and
chiropodists to provide a full basket of primary care services that will
meet the needs of the entire community. The Minden Medical Clinic
currently leases space to Dr. Ben Wu where he operates his family practice
for Minden
clients.
Hyland Crest and Highland Wood are Long Term Care
facilities providing RN, RPN, and PSW services to the residents.
Operated and funded according to Ministry of Health & Long Term Care
standards, the two nursing homes provide optimal care to the community's
elderly folk. Daily activities, nutritional meals, and visiting
entertainers help to ensure well-rounded events for those who choose to
participate, and privacy in the one and two room suites offer rest and
relaxation for those so inclined.
Placement to Hyland Crest
and Highland Wood is provided through the Haliburton, Northumberland &
Victoria Community Care Access Centre. Contact their
Haliburton office at 705-457-1600 or visit their website:www.hnv.ccac-ont.ca
for further details.
